Accessing behavioral health services can seem daunting, but you don’t have to do it alone! With the support of the Colorado Health Foundation, Vail Health Behavioral Health is offering community behavioral health case management services in Eagle County. Behavioral health case managers are trained to walk you through how to access behavioral health services and programs.
  

What Is Behavioral Health Case Management?

Behavioral health case management is a service that helps individuals navigate the complex system of behavioral health care and connects people to available resources and services that best meet their needs when they experience barriers to access. 
 

How Does Case Management Work?

Case managers generally contact clients for the first time through a phone call to introduce themselves and set up an appointment to meet in person. The first in-person interview can take place at a Vail Health Behavioral Health office, in the community or at the client's home. The goals of the first interview include reviewing and completing required documents and assessments. The case manager and client will establish goals and objectives that will guide their work together. The needs and goals of the client will determine how often and for how long case management services are provided.

Case managers meet individually with clients to create engagement and build trust and rapport. During these meetings, case managers identify concerns and barriers that patients are experiencing, identify resources to overcome these barriers, connect people with the available resources and services that best meet their needs and coordinate these services.

Case managers often assist with application completion, submission and tracking, phone calls to set up appointments, arranging transportation and facilitating effective communication between parties. Case managers also help explain complicated systems and procedures that can seem overwhelming or foreign to patients, providing support and assistance in a competent, comforting way. Case managers have backgrounds in behavioral health, but they are not licensed therapists. Spanish-speaking and bicultural case managers are available.
 
Behavioral Health Case Management Services Help Clients:
  • Connect with therapists
  • Access drug and alcohol treatment
  • Understand treatment options
  • Connect with an insurance navigator
  • Explore financial assistance options including:
  • Connect to short- or long-term care
  • Find additional financial alternatives to assist with medical expenses, housing assistance, food assistance, and emergency funding to help with immediate needs
 
Behavioral Health Case Management DOES NOT Provide:
  • Mental health therapy
  • Crisis services 
  • Translation and interpretation (services are available in Spanish)
  • Legal services
 
Please note that behavioral health case management is a non-clinical service. Case managers will provide supportive services and have extensive background in behavioral health, but they are not licensed therapists. 

Behavioral health case managers have offices at the Edwards Community Health Campus and Frisco Health Center. Services are mobile and can be provided at a convenient location for the client.

Diana Bonilla

Behavioral Health Case Manager

As a case manager, Diana helps to navigate behavioral health resources available in the community while also identifying social determinants of health, creating goals, and working alongside her patients to improve overall health. 

Diana was born and raised in Costa Rica and moved to the United States in 2020. She is bilingual Spanish-English and loves to provide compassionate care through a Social Justice lens. Diana enjoys spending quality time with her family and friends, watching series, and reading good books!
 

Gladys Tellez-Villa

Behavioral Health Case Manager

As a case manager, Gladys helps to navigate behavioral health resources available in the community while also identifying social determinants of health, creating goals, and working alongside her patients to improve overall health.

Gladys was born in Mexico and moved to the US in 2000. Raised in the Vail Valley, Gladys enjoys snowy days, camping, and the colors of autumn. She is passionate about raising awareness on equity and social justice issues and practicing patient-centered care. On her free time, Gladys enjoys spending quality time with loved ones, true crime podcasts, crime drama television, and cooking. 
 

Madeline Calel

Community Outreach & Enrollment Specialist

As the Community Outreach & Enrollment Specialist, Madeline Calel helps with new or renewal applications for Medicaid. She is a community-based enrollment specialist. She can also help you apply for various other government financial assistance programs, including food stamps and Cash Assistance; help you navigate other health insurance options, such as Connect for Health Colorado and Mountain Family Health Center’s discount program; and connect or refer you to various other resources and services in our community that support your wellbeing.

Madeline grew up in Leadville, CO. She is passionate about helping the Latin community and practicing language justice meaning creating a safe place where you can speak the language of your heart. Madeline loves to spend time with family and go out on outdoor adventures.
